Medical and health information – what we need to know and why
We want you to feel well looked after at Down to Earth and we can only do this if we know what your individual needs are. It is very important we know about your medical and health needs. This will help us decide how we should adapt our activities and, in rare circumstances, decide if you cannot take part in specific activities.
We need to know about: asthma, diabetes, depression, epilepsy, being pregnant, being allergic to stings/bites or allergies to food types, recent breaks or fractures (last 3 months) or hospital treatment (last 3 months).
This list is not exhaustive so please do tell us about any relevant medical conditions.
Things change, so please do tell us if your medical situation changes after you’ve completed this form and before your time at Down to Earth finishes.
Your personal medication
We need to know if you are on medication in case of an accident – we’d pass this information on to the emergency services. It is your responsibility to bring along the medication you need during your time at Down to Earth – such as an asthma pump or insulin. We reserve the right to prevent you from taking part in an activity if you do not bring your medication with you. Keeping everyone safe is our highest priority.
Other needs
We want you to feel well looked after at Down to Earth and we can only do this if we know what your individual needs are. This can be very wide ranging but may include specific food needs or having Autism or a phobia, for example. If you let us know about these needs, we can do our best to support you.
Thanks for taking the time to read this information! Let us know if you’d like to know more.
